By purchasing this reading, I give Permission to Angelina to connect with my most benevolent guides and ancestors for the duration of this reading to support my highest and best good in the kindest way.
I have received information that Energy Work is a method for assisting the body in its natural healing process. It is a complementary therapy not intended to replace any currently prescribed medical treatments as ordered by my physicians, nor any other medical care I have been advised to seek by them.
I have been informed that Angelina Yokoyama-Teh will neither diagnose, nor prescribe for any condition that I may have, nor does she make specific claims regarding results from Energy Reading and Healing sessions that I receive. I have also been informed that Angelina Yokoyama-Teh is not licensed to practice medicine in any of the U.S. states. I have been encouraged to consult a licensed medical practitioner for any physical or mental complaints that I may have. Treatment/s will not replace conventional medical diagnosis or treatment. I will continue taking medication prescribed by a licensed medical physician and will continue to follow their instructions.
Some of the indications for an Energy Reading/ Healing session include, but are not limited to:

• Ability to make clearer decisions with ease
• Becoming more aware of your role in creating your life
I have been informed that all client information and records are treated in a confidential manner. My experiences during these sessions are confidential and subject to the usual exceptions governed by state and federal laws and regulations.
By purchasing this order, I hereby waive and release Angelina Yokoyama-Teh from any and all liability past, present and future relating to her services. Except in the case of gross negligence or malpractice, I or my representative(s) agree to full release and hold harmless Angelina Yokoyama-Teh from and against any and all claims or liability of whatsoever kind or nature arising out of or in connection with my session(s).
My questions have been answered to my satisfaction regarding Angelina Yokoyama-Teh’s training and experience and what I might expect from my session/s.
